Type: Injury Date/Time of Incident: June 23, 2005, approx 5pm Location: DTW Routing: NW49, CDGDTW Description of Animal: Golden retriever named “Henry” Description of Incident: Upon arrival, agent noted kennel damaged and latch broken. When dog was removed from the kennel, it was observed that his tooth was sticking out and later that four toenails were detached from his paw. Cause of the Incident: Kennel damaged. Cause unknown. Corrective Action Taken: NW contacted vet and made arrangements for tooth to be extracted and toenails to be treated
