Description of the Incident: Self-inflicted injury to the mouth which occurred when animal chewed through the kennel.

Description of the Cause of the Incident: Animal was not acclimated to its kennel and tried to chew its way out. Slightly injured its mouth in doing so. Owner agreed to take the animal to her own vet at her expense.

Description of any corrective action taken: None needed.
