Date: 10OCT2015 Time: 1:15 p.m. Reporting Station: HNL Flight: HA 3 From: LAX To: HNL Animal Information: Description: Female Bulldog Incident: Upon arrival of HA3, the ramp agent noticed that the dog in the kennel was not responsive. He contacted the Lead Ramp agent, and the kennel was taken to the State Animal Quarantine Office. They opened the kennel and the dog was cold and stiff. The owner cremated the dog on October 19. Cause: Unknown Corrective action: Cargo fee and cremation fee refunded.
