Name: Sergeant Major Species: K-9 Breed: English Bulldog Sex: Male
Age: 2 Years 8 Months Color and Markings: Brindle

Narrative Description: Description of Incident: Customer was traveling with her dog, Sergeant Major. The dog was checked in at the ticket counter in Richmond, VA (RIC) connecting in Chicago, IL (ORD) and Los Angeles (LAX) with a final destination of Yuma, AZ (YUM). [Redacted] advised the agent in RIC that her veterinarian stated that Sergeant Major could be excitable and recommended that she give him a Benadryl tablet prior to the flight which she did. Upon arrival in Chicago, IL (ORD) Sergeant Major was deceased. [Redacted] was notified and declined an autopsy and asked that we send Sergeant Major back to Richmond, VA. with her.

Description of cause of the incident: Autopsy was refused therefore cause of death is unknown.

Description of any corrective action taken: None. Trans States procedures were followed.
