Incident 5 Carrier: Alaska Airlines Flight: 806 (Kona, HI to Seattle, WA) Date of incident: Tuesday, July 29, 2014 Time of incident: Approximately 7:42 PM PT Description of animal (including name): A dog, unknown name, age, sex, or breed. Narrative description of incident: Upon arrival in Seattle, WA a dog in its kennel was found with a missing toenail and blood inside the kennel. The dog had apparently been trying to escape its kennel by digging with its paws at the kennel door, which had an attached food and water dish. The pet owner was advised of the situation of the injury, prior to her two dogs being delivered to the claim area, she stated that they would take the injured dog to a vet. Narrative description of the cause of the incident: A dog arrived Seattle with a missing toenail and blood present in the kennel. There is no evidence to suggest that the airline’s handling contributed to the animal’s injuries. Narrative description of any corrective action taken in response to the incident:
