INCIDENT #3: CARRIER: United Airlines flight UA200 DATE: JULY 16, 2014 ROUTING: HNL (Honolulu, HI) to IAH (Houston, TX) to IND (Indianapolis, IN) Approximate Time: 1940hrsLCL DOG: Age 9yrs Breed: Male Boston Terrier Name: Diego UA AWB 016-5322-3435 Owner/Guardian REDACTED NARRATIVE DESCRIPTION Description of the Incident: DEATH Description of the Cause of the Incident: Animal arrived deceased at IND. Boston Terriers of this age and weight are currently restricted; however, United Airlines reserved and accepted the animal for transportation. Although United offered to provide for necropsy to clarify the cause of death, the Pet Owners took possession of Diego at IND. A necropsy report has not been shared with United Airlines. Description of any corrective action taken: Actual cause of death was not disclosed to the carrier. Call Center and origin station were directed to review company resource material which outlines internal policy concerning restricted and embargoed breeds.
